For every premises licensed for the supply of alcohol, a personal licence holder must be specified as the designated premises supervisor (DPS).
The DPS is usually someone that is involved with the day-to-day operating of the premises and as the DPS they are responsible for all sales of alcohol under the premises licence.
No sale of alcohol can take place under a premises licence unless a DPS is specified.
There may be a number of personal licence holders working at a premises but there can only be one DPS.
The DPS must be appointed by the premises licence holder (who may hold both positions).
If the DPS named on the premises licence needs to change, then the premises licence holder will need to make an application to vary the DPS.
How to apply
To complete an application vary the DPS on a premises licence you will need to submit the following:
- a completed vary DPS application form
- a consent form for the proposed DPS to show that they consent to taking on this responsible role;
- a copy of the current premises licence or a statement as to why it is not included
- the prescribed fee, which is currently £23

If you apply online then the licensing authority will serve the application on the relevant responsible authorities. Payment will be taken on submission via the GOV.UK portal.
Email or post
If you apply by email you must use the application forms provided below and not the PDF forms located on the GOV.UK portal. If you apply by email then the licensing authority will serve the application on the relevant responsible authorities.
